This page is here to assist you.  It allows you to view many different informational sheets, whether they contain pre-op instructions, post-op intstructions or our standard Patient Information and Medical History sheets.

If you are a new patient you can print and fill out our Patient Information sheet, and Medical History sheet and bring them with you for your first visit with Dr. Frantz, Debbie, or Kari.  This saves you time and allows you to relax in our comfortable reception area rather than filling out paperwork.

If you are about to have surgery by Dr. Frantz, or you have already done so, we have all of our Post-Operative Instruction sheets available for you to view anytime you wish.  We give you a hard copy as part of your pre-operative visit so you know what to expect after surgery, but if things get lost in the shuffle of life, you can always view them here on our website.

We also have a number of other informational sheets for a variety of different illnesses that we commonly treat in the office.  There are also a few questionnaires that you can fill out to better assist the doctor in your treatment.
